Staff Nurse for Family Practice Office. Responsibilities include, but not limited to, assisting patients to an exam room,
updating their medications and medical history and testing for individual suspected diagnosis such as Flu, Strep, UTI's,
Chest Pain, etc., prior to the doctor's visit. Assisting the doctor in all procedures in the office. Answering phones and
assisting patients with their medication or medical needs, making referrals to Home Health Agencies, Home Medical
Equipment and any other needs in the realm of Family Practice.
2004-2008 LEXINGTON CLINIC
Registered Nurse, Float Pool. Worked with a team of nurses to provide needed coverage throughout the entire
Lexington Clinic, for vacation, maternity leave, sick leave, etc. Coverage include areas of Family Practice, Internal
Medicine, Endocrinology, Urology, ENT, Pulmonology, Cardiology, Dermatology, Rheumatology, Ambulatory
Surgery, Lab and outlying Family Practice offices.
1998-2004 CASE MANAGER, SAMARITAN HOSPITAL
Case Manager of Medical-Surgical floor. Responsibilities include but not limited to Case Management of Medicare,
Medicaid and Private Insurances, utilization review, discharge planning, social issues and financial issues of all
patients.
1997-1998 CLINICAL MANAGER, CARETENDERS OF THE BLUEGRASS
Managed a team of medical-surgical nurses and nursing assistants for home care services. Coordinated home visits for
a group of patients, provided communication between all disciplines, and assisted staff in case management of
individual customers.
1996-1997 NURSING COORDINATOR, AMERICAN NURSING CARE
Responsible for full-time and PRN nursing staff coordination of patient care. Scope of job included facilitating
communication between all disciplines to provide total quality care, organizing in-services for clinical staff, providing
orientation to new employees, and assisting staff in case management. Knowledgeable in Medicare and, Medicaid
regulations. A member of Corporate Skin Care Team. Was appointed Interim Manager of Clinical Services in July,
1997.
1992-1996 REGISTERED NURSE CASE MANAGER, AMERICAN NURSING CARE
Four years experience in Case Management of the Medicare, Medicaid, Insurance and Waiver processes. Extent of job
included a thirty to forty client assignment to manage complete home care needs as well as communication with all
disciplines involved. Extensive knowledge in the management of clients with diabetes, acute and chronic wounds,
catheter maintenance, and other varied disease processes.
1991-1991 REHABILITATION SPECIALIST, INTRACORP
Worked with various insurance companies involving workers compensation cases. The position included
accompanying clients to their doctor’s appointments to facilitate their rehabilitation process then reporting back to the
insurance companies.
1989-1991 PRIMARY NURSE CASE MANAGER, CARDINAL HILL REHABILITATION HOSPITAL
Three and a half years experience in Case Management of the rehabilitative patient. Responsibilities included
instructing clients and families in the disease process, medications, community resources, equipment and adapting the
home environment by coordination with various home care agencies needed upon discharge.
1987-1989 STAFF NURSE, CENTRAL BAPTIST HOSPITAL
Two years experience as a registered nurse on a medical-surgical floor. Responsibilities included post surgical
monitoring, intravenous catheter maintenance, diabetic monitoring, surgical and chronic wound care, gastric and
urinary device maintenance and peritoneal dialysis. Assisted on Orthopedic, Diabetic, Telemetry and Women’s Care
floors as needed.
