Professional Experience

2011- Present: Medical Records Director, Montgomery County Mental Health Treatment Facility. Responsible for medical record storage and retrieval. Manage all aspects of medical record function and staff including: establishing protocol for quality and risk improvement; quality analysis; comprehensive/periodic/qualitative reviews of active and discharged charts for compliance with state regulations and licensure; certifying all medical records are coded, classified and filed accurately and in a timely basis; completing all ROI requests; identifying and addressing any technical errors, potential legal and/or clinical issues; providing input and feedback to individuals of the multidisciplinary team regarding documentation deficiencies in a patient’s chart and/or informing quality, risk and performance improvement administrator of pertinent issues. Additional duties include: privacy officer for facility, training all personnel on EMR and HIPAA, working hand-in-hand with physicians and all disciplines to ensure data integrity of EMR input, and serving on various hospital committees including medical records, forms, performance improvement, and environment of care. Avatar is the EMR at this facility and I worked closely with IT during the implementation process.

2009-2011: HIM Director/Privacy Officer, Promise Hospital Nederland, TX. Responsible for concurrent audits of all patients charts, ICD-9 Coding, utilizing MediTech to track charts and obtain data to submit for State reporting, orientation of all new hires on Medical records process’ and training on HIPAA, processing all delinquent records, working closely with all physicians to remain compliant with entries in charts, and training all physicians on proper documentation in a medical record. Ensured Medicare compliance was maintained, led forms committee and was a member of Performance Improvement and Environment of Care committee. 3M was this facilities encoder system.

2009: Internship, Memorial Hermann Northeast. Coding of inpatient, outpatient and Emergency Room records; auditing of charts for proper coding and compliance with Joint Commission requirements; attended forms, medical records, and quality committee meetings; used 3M encoder
