1-House officer in Tanta University Hospitals from March 1st 2006 to February 28th 2007.
On the house officer-training year, I have passed through training programs imposed by each department of Tanta University Hospitals (practice was under complete supervision of the departments’ staff directly or through the residents’ notes about our practice & achievement):
1. Department of Surgery:
• In the Emergency Room E.R : I have managed Polytraumatized patients since their entrance to the hospital through resuscitation & stabilization of the patients towards their admission & in-patient management ,also I have practiced wounds stitch with its all ordinary types (simple interrupted & continuous , mattress either horizontal or vertical )
• In the Operation Room O.R: I have practiced minor operations like circumcision & abscess drainage. In addition, I have assisted in major operations like appendectomy, cholecystectomy, hernia repair, spleenectomy, above knee amputation & abdominal exploration.
• In the patients’ wards: I have practiced postoperative management & follow up of the patient condition until they are discharged.
• In the clinic: I have practiced maintenance follow up & treatment of the patients after being discharged from the hospital, I have also practiced management of cold surgical cases like uncomplicated piles & hernias.
2. Department of Internal Medicine:
• In the emergency room: I have practiced management of medical emergencies like Diabetic Ketoacidosis “D.K.A”, hepatic precomas , renal colics & asthmatic attacks.
• In the patients’ wards: I have practiced follow up of the patients treatment & their case progression, beside that I have practiced complete management of a hepatic patient in the hepatic unit, in which we deal with either patients with hepatic precomas or patients with active bleeding varices in which rapid & effective intervention must be put in action to save patients’ life, procedures we trained on in the hepatic unit are intrajugular cannula positioning & paracentesis.
• In the clinic: I have practiced management of ordinary medical complaints & problems like GIT upsets , GERD, Respiratory tract infections & chronic asthmatic cases , dysuria & change of urine color , these are the most common cases I faced to in the clinic.
3. Orthopedics Department “special surgery training”:
• In the E.R: I have practiced management of the bone fractures of all types, I have managed multiple cases of scphoid bone fracture & clavicular fractures, I have done bone casts, slabs & fracture banding.
• In the O.R: I have assisted in reduction of fractures under anesthesia, house officer were not allowed to assist in more advancing operations.
• In the Clinic: I have practiced management of rheumatoid arthritis, gouty arthritis & osteoarthritis, beside that the follow up & management of complications of fractures castings were achieved in the clinic.
• In the Wards: house officers under supervision of residents did follow up & maintenance of treatment of the postoperative patients.
4. Neuro-Psychiatry Department:
A) Neurology Section:
• In the E.R: I have practiced management of acute brain strokes either hemorrhagic or ischemic; also, I managed acute epileptic attacks.
• In the neurology ICU: I have practiced admission of the patients to the ICU with full treatment prescription, cases I admitted most commonly are strokes.
• In the Clinic : Because of medico-legal issues concerning the drugs prescription for neurological & psychiatric patients, House officers were not allowed to mange the neurology & psychiatry cases in the clinic alone , they were only allowed to attend the clinic as observers, neurology cases I most commonly have seen are Parkinsonian cases & Chorea cases.
B) Psychiatry Section:
• In the E.R: I was faced to hysterical cases like hysterical blindness & hysterical muteness, also I was faced to neuroleptic drugs complications like acute dystonia & tardive dyskinesia .I have seen Tics cases beside the previously mentioned cases.
• In the Clinic: As mentioned above, because of medico-legal issues concerning the drugs prescription for neurological & psychiatric patients, House officers were not allowed to mange the neurology & psychiatry cases in the clinic alone , they were only allowed to attend the clinic as observers, Psychiatric cases I have seen most commonly were , Depression, Bipolar
& manic patients & Schizophrenic patients.
• In the Wards: House officers under supervision of residents did follow up & maintenance of treatment of the admitted psychiatric patients.
5. Obstetrics & Gynecology “Ob/Gyn” Department:
In the Ob/Gyn Dep. I was (as a house officer) most commonly managing normal labor cases under complete supervision of the residents. Beside that, I observed many C- Section operations as House Officers were not allowed to assist in the Ob/Gyn operations. In the clinic, house officers were only allowed to attend as observers, I have seen most commonly complete management of the genital Tract infections & Birth Control strategies
6. Pediatrics Department:
• In the E.R: I have managed cases of acute diarrhea with full fluid replacement therapy in the dehydration section. I have also managed poisoning case with gastric lavage technique & activated Charcoal. I managed acute asthmatic children & children with respiratory tract infections. Beside that, I have managed hyperthermia fits.
• In the clinic: I have managed ordinary GIT upsets, uncomplicated upper respiratory tract infections, uncomplicated urinary tract infections, anemic cases “most commonly due to iron deficiency then thalassemia” & parasitic infestations. Follow up of the discharged cardiac cases were admitted in the clinic. Neuro-pediatric cases are either discovered in the E.R or as delayed milestone complaints by mother of the patient, in such case house officer was obliged to direct the case to the residents or higher staff members to admit.
• In the Wards: In Pediatric department, wards were categorized into neurology wards, blood diseases, leukemia & lymphoma wards, diarrhea wards, cardiac wards & respiratory tract wards. House officers were allowed to manage the follow up & maintenance of the treatment of the in-patient cases. Beside that, in leukemia & lymphoma section we were allowed to attend & observe intrathecal chemotherapy for the CNS involved Lymphoma cases.
2-General Practitioner in the Ministry of Health from March 1st 2007 until October 2007.
The practice of general practitioner is done in the rural health units following the ministry, Tasks of physicians are filing of the patients visiting the health unit clinics, reporting of any epidemic disease newly discovered & referral of patients in-need to the Central Hospitals.
3-Resident of Anesthesiology in Benha Educational Hospital, (Benha City, Egypt) since October 2007 until September 2008 : In my residency I have practiced general anesthesia in healthy patients, cardiac, eclampsia pregnant patients & hepatic patients; the practice was with both manual anesthesiology devices & computerized devices. In addition, I have practiced spinal anesthesia, regional anesthesia & nerve blocks.
4-Resident of Anatomical Pathology in National Egyptian Bank Hospital, since July 2010 then in Nasser institute of health & research since September 2010 tell now. I decided to change my specialty after I accompanied my father to one of his labs ((my father is a Professor of Pathology in Tanta university Hospitals ( Tanta City , Egypt ) & King Abdul-Aziz university Hospitals (Jeddah ,Saudi Arabia)) . After I saw specimen cutting, processing & Pathological slides reviewing in the lab I got a deep interest in pathology, which is translated into reapplication to residency position matching in Egypt.
In the department, residents have intense experience and gradually increasing responsibility in the various disciplines of anatomic pathology. Close daily contact with the attending staff, often one-on-one, assures close supervision and continuing evaluation. Residents are expected to attain the basic and practical knowledge of pathology. Intellectual and scientific aspects of pathology, including pathogenic mechanisms and clinico-pathologic correlation, are emphasized in didactic conferences.
Our duties as pathology residents are as following:
1- Description & Cutting of the received specimens prior to submitting them into the processing & staining by technicians.
2- Reviewing of pathological slides under supervision of the department Professors to learn how to read the slide , describe the microscopic pictures & diagnose cases in the right way in a professional & competent manner.
3- Supervision of the secretarial work of the department, registration of cases & handling the reports.
