Presently, working as Medical Transcriptionist at The Mission Hospital, Durgapur (from 05th October, 2009 till date), for 2 years 6 months.

Work Experience:

More than 3-1/2 years of working experience as a Medical Transcriptionist.

Previously, was associated with Infovision Software (A division of SSAPL) [ITES], Gariahat, Kolkata, as Medical Transcriptionist (From July 21st 2008 till October 2009).

Current Roles & Responsibilities:

Preparing Discharge Summaries, Case Summaries, Death Summaries, ER Note, Progress Note, Initial Consultation, Treatment Summaries, Cath-Lab Reports, Emergency Room Reports, Operative Reports, PTCA Reports, Psychiatry Reports and various other types of Medical Documentations for the patients, hospital records and Insurance/Corporate Companies by own self or from Consultant’s/RMO’s script or dictation.

Swiftly and efficiently managing around 35-40 (or more) patient discharges per day, thereby, reducing the turn-around-time (TAT) for patients, thus, reducing the rush and making sure of the availability of vacant beds for newly anticipated patients.

Understanding of a patient’s medical condition and treatment rendered, by going through the patient’s progress notes, in order to prepare documents by own self.

Getting the documents checked and under-signed by concerned Consultant/RMO.

Handling the patients and their attendants, taking care of their plans and thereby, reducing their time and grievances regarding delayed discharge process.

Assisting the HOD in day-to-day responsibilities of the department. Preparing monthly duty roster.

Preserving and maintaining day-to-day documentation/records/progress notes of patients in an easy accessible and systematic pattern.

Confronting with Consultants/RMOs to obtain accurate information regarding patient’s status for documentation.

Designing templates/formats according to international standards and making them easy to work with.

Taking care of the availability of all kinds of medical document/data to the patients and for hospital (record).

Counseling of patients and their attendants, and answering to their document-related queries.

Issuing Fitness Certificate, Sick Certificate, Transfer letter by self (as per Consultant’s order).

Medical Transcription Skills:

Transcribing with speed of approximately 65-70 lines per hour (50-60 words per minute) / 600-650 lines per day.

Thorough knowledge of medical terminologies related to Anatomy, Physiology, Surgery, Pharmacology, Laboratory, Disease Processes, Equipments, Medicines, etc.

Extensive knowledge of human physiology, human anatomy and understanding of clinical and disease management.

Dealing with multispecialty files such as Gastroenterology, Orthopedics, Neurology, Infectious Disease, Rheumatology, Cardiology, Dermatology, Psychiatry, ENT, Hematology, Ophthalmology, Oncology, Gynecology etc.

Dealing with various work types such as Discharge Summaries, Treatment Summaries, Case Summaries, ER Note, Progress Note, Initial Consultation, Death Summaries, Cath-Lab reports, Emergency Room reports, Operative Reports, PTCA reports, Psychiatry Reports, etc., as well as preserving them in an accessible and systematic pattern.
