Direct and implement resident’s continual therapy and completing Physical Therapy,

Speech Therapy or Occupation Therapy programs. Entailing the continuation with

whatever residents requires or needs using trained Certified Nursing Assistance. Assess

all residents after admitted to facility with risk for falls, consisting and focusing on

resident’s safety. If falls occur implementing plan of care, follow thru via

communication to Charge Nurses of any changes made.

Restraints implemented after falls of three or greater start with lease restrictive per policy.

Meet with Power Attorney for resident to discuss restraint to be used and obtain signed

Consent. Weekly Falls and Restraint meetings with Physical Therapy Director and

Committee members. Information on each resident updated on computer ongoing along

with daily updating on Pocket Care Guides.

Direct the Keep off of Floor programs.

Monitor all infections and or new medication ordered by MD each month input

information into computer after determination of these new orders for antibiotics meets

facilities policies to be determined true infections.
