1. Chaired Daily Neurological Meetings with Multidisciplinary Tea

2. Implemented a System To Identify Any Patient in the hospital who was exhibiting signs and symptoms of a TIA or Stroke.

3. Educated pt/families about the 5 Points of Stroke at least twice per patient (per JCAHO Requirements) and Documented all Education Completed.

4. Supported /Guided and was an Advocate for the patient/family.

5. Audit TIA/Stroke Medical Charts for Completion: Education Started/Documented, Medications Ordered, Consultants Ordered, Dysphagia Screening, ST, PT, OT, SW, etc.

6. I Implemented a Role Out to the Leadership Team at Virtua regarding Stroke Education, before I left. The Ultimate Goal was to have the Staff Accountable for Education and Documentation.

7. I Effectively Re-Established our Certification in Stroke because of being 100% Compliant in Education and Documention (100% Effective from the beginning to the end, a total of 8 months.)
