Ability to collect, analyze, enter and report care management data related to patient care and participate in continuing quality improvement.
Ability to identify discharge planning needs of patients through chart review, patient/family and multidisciplinary team review.

Utilization Review Skill Set
Able to review patient medical record for appropriateness of admission and/or continued stay using established criteria and communicate utilization issues to clinical team, business office or other appropriate parties.
Coordination of follow up activities related to Medicare, other third party payers and negotiated contracts.
