Instructed members regarding MD treatment plans, medications, DME use, follow-up visits with PCP and other MD’s and encouraged compliance. Referred appropriate members to Chronic Care Case Management program as needed. Referred members to in-house MSW as needed. Triaged members who called in with medical problems and placed conference calls to appropriate MD or called 911 for members if necessary. Arranged transportation for members to and from MD for visits and to hospital for surgical procedures. Assisted members in obtaining timely follow-up visits with MD’s as needed. Assisted with training new case manager team members.*I left to take of a sick family member from 11/09 to 10/10 when I returned to work full time*

Performed assessments and follow-up’s for Workmen’s Compensation injuries.*The company was sold to Concentra and I could not work nights.*

Part of a two supervisor team. Responsible for scheduling daily assignments for all field nurses; reviewed and corrected all Medicare OASIS documentation; ensured all Medicare guidelines were followed by office staff and nurses. Other duties included acceptance of appropriate new patient referrals; daily collaboration with MD’s regarding patient care; performed patient chart audits according to Medicare criteria; educated and trained field staff as well as clerical staff to ensure newly admitted/existing patients had necessary DME in place; scheduled time off for staff per their requests as well as counseled staff where appropriate. *The supervisor team was cut to one person who was also expected to make field visits-I declined the offer of supervisor*

Began employment in Day Surgery; responsible to pre-op and post-op care of day surgery pts. Duties included assessments, IV starts and readiness for surgery. Post-op duties included monitoring pt’s vital signs, pain levels, recovery from anesthesia, readiness for discharge and post-op teaching to pt./family member. Surgeries ranged from endoscopy to orthopedic procedures such as knee and shoulder scopes. Pt. populations ranged from elderly to pediatric. November, 2003, transferred to Medical/Surgical/Telemetry Unit as staff nurse due to highly erratic hours in Day Surgery Unit. Duties included direct, total pt. care; admissions and discharges; IV starts, IV medication administration, TPN, insertion and monitoring of NG tubes, blood and blood products administration, monitoring of cardiac pt.’s, PEG tube care and feedings, wound care, post-op care, and PCA pump management.*I left because I needed to change to the day shift and no positions were available*
