Provided support with medication management for comfort, ordering supplies needed, family/staff education regarding hospice care, psychosocial support in consultation with MSW, managed and help support bath aides in the home/ECF setting, consulted and collaborated with primary care physicians, pharmacists, nurse practioners, and volunteers. I assisted in community relations on the San Juan Islands and cared for patients on Friday Harbor, Lopez, Orcas islands as well as Anacortes.
