• Piloted the Screening Youth Program, a continuation of SCY, focused on providing mental, social, emotional health screenings and case management services to families with children exhibiting signs of behavioral/mental health struggles.
• Self-managed an active caseload of 15+ families across Fairfield County and collaborated closely with internal/external agency programs, as well as state and local agencies, to connect with families in need of comprehensive, supportive case management services.
• Conducted 350+ unique screenings & assessments for a total of 210 families/individual caregivers in Fairfield County and facilitated access to appropriate services by identifying needs and addressing cultural, social and economic barriers to accessing services.
• Performed regular home visits for families in order to foster a strong client-provider relationship and ensure familial health & wellness.
• Functioned as an on-site crisis case manager for the Ferguson Library in Stamford and provided long-term case management services for a total of 75 patrons of the library (ranging widely in age, sex, race, ethnicity and socioeconomic status) experiencing hardships or in need of assistance with a plethora of different issues (housing access, food insecurity, job assistance, education, etc).
