Below is important information regarding your separation and benefits. Please note that except as noted below, all pay and Companyprovided benefits will end at midnight on your separation date. The details of your pay and benefits are outlined below:
Pay
Your final paycheck will include all wages earned through your separation date. In accordance with company policy, accrued, unused vacation, sick or personal hours will not be paid out unless required by applicable state law, client contract or collective bargaining agreement. If you have any questions regarding final pay or local requirements, please contact your manager or HR using the information at the end of this letter. W-2 and Paychecks

Insurance Coverage
If you have company-sponsored health benefits, this coverage will automatically terminate at the end of the month on your separation date. Continued coverage will be available thereafter, provided you pay the entire cost of the coverage. You will receive notification of your right to continue your insurance coverage through COBRA. This will be mailed directly to you from our COBRA provider at the home address we have on file. You must apply for coverage continuation within 60 days of your termination or the date of the COBRA notice, whichever is later. You then have 45 days from the date you elect coverage to pay all the premiums back to when you would have lost coverage. Coverage will then be effective retroactively to your separation date. Additionally, if you have Company-provided group life insurance, or additional voluntary life insurance for yourself and your dependents, these plans are eligible for continuation as well. Information regarding conversion and portability will be mailed to you at the home address on file. Submission of your application and payment of your first quarterly premium is required within 62 days from the termination of your coverage, as noted above. You may contact New York Life directly at 1-888-***-**** for more information. 401(k) Plan
If you currently participate (or have previously participated) in our Company sponsored 401(k) Retirement Savings Plan, please contact our Plan Administrator, Empower, directly to discuss your options regarding rollover or other distribution/investment options. They may be reached at 844-***-**** or www.empowermyretirement.com. Employee Separation Letter – Page 2
