• Engaging in the development of a quality plan to improve in the areas of documentation, service delivery, safety, and more.

• Review individualized support plans (isps) for quality and provides feedback to ensure that documentation complies with Department of Mental Health standards.

• Review monthly documentation to ensure it meets quality standards/Audits.

• Conducts isp, documentation, and focus training.

• Complete monthly visit assigned in the filed for home interviews on the program’s performance and support.

HOURS: 9AM-6PM

RESOURCE FAMILY PARTNER-PENNYLANE CENTERS

• Help families access services and provides linkages to needed support services.

• Helps families increase their advocacy skills for families to obtain services and support.

• Assists families in increasing their support network on behalf of the needs of the child(ren).

• Serves as a role model and mentor for parents/caregivers whose children are receiving treatment services.

• Participates in mental health treatment teams as an equal member to strongly represent the parent’s/ caregiver’s perspective.

• Assists in the ongoing development and evaluation of the child’s treatment plan.

• Provides case management, such as linking children and families to culturally and linguistically appropriate community resources and/or coordinating contacts with other 2

service providers, including, but not limited to, schools, Social Service Agency, and other mental health providers.

• Participates in related trainings and workshops. EDUCATION
