Position: In-Home Services Residential Program Coordinator
This position required contact with community members with special needs, and
their families, as well as with representatives of other companies. This position also required that I advocate for the needs and well- being of the individuals served. My duties included gathering personal and medical information on each service recipient on an annual basis. Information gathered was compiled into an individualized plan designed to achieve optimal growth by the persons served. Responsible for monitoring the progress achieved based upon the progress made for each quarter of the year, additionally responsible for collecting data and compiling plans to monitor high risk and health related issues. This position required excellent communication skills, both written and oral, as well as extensive customer service skills and maintenance of many confidential issues.
