My duties within Transitions were to assist in scheduling all social workers, chaplains and bereavement specialists to all patients. I handle all referrals and medical records needed to find placement for patients that are no longer able to be tended to at home. I ensure that all guidelines placed by Medicare are met and that all HIPAA guidelines are as well. Prior to working with the 25 social workers and chaplains, I maintained schedules for all our nursing personnel more than 60 persons at a time. I also create and update daily our 5 day admission audits, 10 day post bereavement audits and our volunteer visit reports. HCHB, Wellsky and Forcura were programs used.
