Thousands of paper documents are scanned every day to allow them to be attached to electronic medical records.at university of Utah Health, scanning is decentralized, meaning paper forms are scanned at the clinic in which they were received. However, attaching this document to the patient charts is preferred by the health information team, who review incoming documents in the Document Management System.
