As a Community Healthcare Worker I work collaboratively with the Medical Providers and assist/support patients with community resources that are needed to help them with stability in the community. I help connect patients with food insecurities, housing supports (rental assistance), connecting them with community partners for assistance, collaborating with Care Oregon for assistance, submitting referrals for a variety of needs such as, assisting with housing needs for individuals who do not have housing, assisting with transportation needs, assisting with ensuring safety needs and collaborating with community partners to establish safe housing.
