Clinical Experience (Past to Current)

Chautauqua County Home, Dunkirk, N.Y. 5/1988 – 12/2014

LPN (1988-1991) – Worked the medication carts dispensing medication and injections to residents. Worked the treatment cart handling resident care for treatments. Assist with bathing and activity of daily living with the residents. Each unit had 44 residents and we had 5 units.

RN Unit Nurse (1991-1998) – Managed staffing for unit. Set up assignments. I did staff audits of care. Made arrangements for resident appointments, resident care meetings, MDS for residents. MD rounds and MD orders. Worked as Supervisor of building on weekends. Blood draws and IV’s. All duties related to resident care. Care planning and family meetings.

Inservice Coordinator (1998-2012) – Schedule in-services for 300 staff members. Track all department mandatory in-services by arranging and conducting them. Set up and conduct all staff immunizations, Hepatitis vaccines, flu vaccines, pneumonia vaccines, and annual health assessments. Bi-annual auditing of nurse aide routine care of residents. Ongoing auditing of monitoring of units, rooms, and resident care. On spot in-services as needed. Create policies and procedures and review of current policies with all staff. Implementation and in-servicing of new policies. Interviewing and hiring of new employees. Arrange and conduct all new employee orientation and MD physicals as well as monitoring of job performance after employment. Teaching of IV and PICC lines to all nursing staff. Periodic coverage as needed for wound care, treatments, and medication passes. I also headed up the Safety Committee for 10 years ensuring the MSDS Book was maintained and all staff in-serviced yearly with monthly meetings held with all department heads. Also functioned as a PRI Assessor for many years and still certified as a PRI Assessor.

Certified Nurse Aide Program Instructor (1994- 2014) – Interviewed candidates for nurse aide program. Conducted classes which are 3 weeks in length. Classes are 40 hours a week of classroom and clinical with frequent exams and a final written and clinical exam for passing. I have taught approximately 50 classes over the years. After the course I am responsible for meeting with the employees throughout the next year for auditing of care.

CPR Instructor (1998-2014) – Responsible for conduction classes in CPR for healthcare professionals at our facility which was approximately 75 nurses. Maintain credentials for current CPR and nursing licenses.

MDS Coordinator (9/2013-12/2014) Acting Director of Nursing (2/2014-1/2015) – Retained both titles and job duties until the sale of the facility to a private organization January 1, 2015. As MDS Coordinator I was responsible for setting up all MDS schedules and reviewing insurance information on potential incoming residents. I handled all of the reimbursement for residents, Medicare and Medicaid eligibility, IPRO appeals for families, and HMO coverage throughout their stay. I monitored medical condition and documentation for Medicare and HMO eligibility and issued ineligibility cut letters to residents and families. I scheduled and oversaw all weekly meetings with social workers, nurses, PT, OT, and speech therapies for updates to Medicare and HMO. I handled and organized the MDS schedule on residents for reimbursement status as well as completed them myself. I conducted PRIs prior to the implementation of the MDS. As Acting Director of Nursing, I maintained the everyday workings of the facility until our last day.

Visiting Nurses Association, Jamestown, N.Y. 5/1994 - 11/1999

Duties included handling client cases in their homes. I taught medication, did nursing assessments, administration of medication and insulin, glucose checks, wound care, and medication orders. I did outpatient teaching. I attended meetings and interacted with social workers and the MD on resident care. I set up home health aides and their schedules according to patient needs. I monitored the safety environment in the home and created care plans for the patients.

Lakeview Shock Incarceration Facility, Brocton, N.Y. 10/2003 – Retired 2/19/2023 from State Service with 30 years.

I started as a Per- Diem Nurse in 2003 then became fulltime in 2014 when The Chautauqua County Home was sold and my employment ended with the county. My job duties include setting up nurse sick call. Seeing inmates for sick call and emergencies, seeing and processing inmate indrafts, outdrafts with paperwork, review inmate charts. I administer flu shots, PPDs, Hepatitis shots, HIV testing, EKGs, and mess hall clearances. I assist the MD and NP on rounds with inmates, administer meds as ordered, draw labs and set up labs and clinics. I attend in-services as scheduled, set up callouts of inmates for dental/labs/sick call, assess inmate medical needs. I assist with diabetic clinic, asthma clinic, hypertension clinic, glucose checks, administer insulin, and schedule follow up appointments as needed. I also do CMSs for parole board and health transfers for inmates as needed.

Collins Correctional Facility, Collins, N.Y. 4/2016 – Retired 2/19/2023 from State Service with 30 years.

I work as an Extra service Nurse performing the same duties listed previously at Lakeview.

Elderwood Skilled Nursing Facility, Hamburg, N.Y. 10/2022-Present

I conduct General Orientation with all incoming new Employees. I am responsible for all Fit-Testing, PPD’s, and Physicals of staff, initial and annually. I set up all mandatory in-services in Relias and monitor for compliance and completion. I am the Director of the Certified Nurse Aide Program. I am the CPR Instructor for the facility. I conduct all auditing throughout the facility with staff, such as incontinent care audits, feeding audits, medication audits, treatment audits, colostomy care audits, and so forth as needed and yearly. I conduct competency skill audits on all nurse and nurse aides. I track and record all vaccines, Influenza, Covid Boosters, and Pneumonia for staff and administer as needed. I also ensure CAN’s complete their 12 hours yearly of in-service time to maintain their certification. I work with the Wound Dr. in his rounds of residents with ongoing wounds and monitor for improvement or decline. Also responsible for conducting safety audits and reporting to Safety Committee.
