Outreach Specialist

Actively recruit families to enroll in evidence based home visiting program across the parishes served by teams of home visitors.
Perform appropriate intake/screening procedures to connect families to the most suitable region or parish specific home visiting team for voluntary enrollment and/or referrals to other community maternal and child health services.
Conduct outreach activities designed to create awareness about the MIECHV program and increase number and type of referral sources. Outreach activities include public presentations, participation in community events, and one on one engagement with service providers and community stakeholders that serve eligible families and potential clients.
Cultivate positive relationships with relevant organizations, social service providers, and community partners. Participate in appropriate community collaborations.
Coordinate and implement MIECHV program events that include relevant community partners; events may consist of client graduations, group connections, and resource fairs.
Inputs intake / outreach data in a timely and accurate manner into the designated OPH data system; complies with or assists in the compilation of information for reports as needed.
Prepares and maintains program related forms, specifically those related to outreach and recruitment.
Complete and submit regular outreach itinerary of projected activities to team supervisor and ECS Integration Specialist.
Submit monthly report related to outreach activities to team supervisor and ECS Integration Specialist.
Participate in monthly outreach team meeting.
Attend regional home visiting team meetings, as required by team supervisor and ECS Integration Specialist.
Display excellent communication skills including presentation, persuasion, and negotiation skills required in working with clients and coworkers and including the ability to communicate effectively, work with people of diverse social, economic, educational, ethnic and cultural backgrounds
Proficiently operate standard office equipment, including computer hardware and software applications. (i.e., Microsoft Office, Word, Excel, Windows, PowerPoint and Access).
