Re: Separation of employment
Dear Donna,
The current client program you are working on has experienced a reduced need for our services. Unfortunately, we do not have work availability on another client program to offer you at this time. As a result, we are processing your separation of employment, effective March 17, 2025, due to a Reduction in Staff needs. We appreciate your contributions to our company and wish you the best in your future endeavors.
Payroll Information
All final wages owed to you, including any days worked through the date of your separation and any will process through your direct deposit account in accordance with state regulations for final pay. Your access to the ADP Workforce Now site (https://workforcenow.adp.com) will remain in effect up to 3 years after your separation date. Please update your personal email address today in ADP portal so that we can send communications regarding payroll and other pertinent information after your separation. Please ensure you update ADP Portal with your mailing address if you move in the future, to ensure you receive year-end tax reports. Benefit Information
If you are currently enrolled in Medical, Dental and/or Vision plan with cxperts, you will have coverage until the end of the month of your termination date. All other ancillary benefits will terminate on the date of your termination. If applicable, you will receive separate written notification of your rights under COBRA to continue your health coverage from cxperts at your own cost. All attempts will be made to recoup any pending benefit premium payments from the final paycheck to ensure coverage until end of the month. However, if it is not possible to recoup the balance in the final paycheck for any reason, you will owe the balance to cxperts. Be advised that in such a case cxperts reserves the right to cancel your insurance coverage for non-payment and will notify you with a fifteen (15) day cure notice in advance of cancellation. COBRA
Under the Consolidated Omnibus Budget Reconciliation Act (COBRA), you and your covered spouse and/or dependents have the option of continuing your current group medical, dental, and/or vision coverage for a period of up to 18 months (or possibly longer with other qualifying reasons), provided you actively elect to do so within 60 days following your separation Date. You will receive a COBRA Notice letter and a COBRA Election Form at your home address within 14 days of your separation date. If you are enrolled in benefit plans, you are eligible for COBRA continuation of coverage.
