Conduct comprehensive assessments and develop an individualized discharge with the patient and their families.

Identify and coordinate access to community resources and services. Educating patients on their rights at discharge. Ensuring they have the medical equipment and services for care at home.

Regularly monitor /re-evaluate progress and medical readiness for discharge. Identify barriers to discharge, insurance authorization, and meeting Medicare guidelines.

Conduct follow-up assessments, identify new discharge needs and adjusting plan as needed, and documenting changes in clients’ conditions or circumstances.

Identify the barriers that will delay discharges when patient is medical cleared. Complete online state PASRRs and PAEs. Prioritize tasks effectively to manage a caseload efficiently and access the physician advisor if needed.
