EXPERIENCE:

Fidelis Care New York (New York State Health Organization), December 2014 to September 2016. I was then placed on Social Security Disability due to two Surgical Back Procedures.

Summary of Position:

The Intake Specialist works closely with a team of health care professionals to help develop a plan of care for members enrolled in Fidelis' Managed Long Term Care program. The Intake Specialist is responsible for conducting assessments and reassessments of applicants and enrollees using the Uniform Assessment System ('UAS') and will be responsible for developing individualized proposed services within the plan's benefit package. The Intake Specialists are expected to use critical thinking and sound clinical judgment when making recommendations for proposed services.

Responsibilities:

I conducted in-person and hands-on assessments and re-assessments of applicants and enrollees;

I completed internal and external documents and information for data submissions to local Departments of Social Service (LDSS) and the Department of Health (DOH) prior to established dates due; creates and modifies plans of care that include authorized services, need based non-covered services, family, and caregiver support to meet identified treatment goals within specified timeframes;

I communicated with patients, family, and medical personnel to facilitate a desired outcome;

I collaborated with providers to ensure appropriate outcomes;

I practiced and adhered to departmental and State guidelines and clinical standards of care;

I managed electronic patient files to document patient records, treatments, and progress.
