- Completes all Terminations and proper documentation.
- Participates in ongoing staff education and in weekly and monthly staff meeting to assure care coordination plan management.
- Works in conjunction with Intake Specialist, Administration Coordinator and Screening Specialist tomaintain CCU care management functions.
- Follow all agency policies and procedures.
- Maintain HIPAA compliance.
