POSITION : NICU STAFF NURSE

JOB DESCRIPTION

• Receives endorsement on time as scheduled, give comprehensive endorsement to succeeding duty, all the necessary data relative to patients' care and status for continuity of care.

• Admits and assess the general condition of newborn, identifies problems, plans, executes and evaluates care.

• Checks vital signs of newborn as per need or indication.

• Initiate thermoregulation on newly admitted infants and maintains it.

• Administers prescribed medications orally and/or by injection observing the 10 Golden Rules and as authorized by law RA 6136.

• Observe, records and reports newborn's condition; reaction to drug treatments and other significant incidents to the junior consultant on duty or to attending physician.

• Administers feedings on newborns via feeding cup or nasogastric tube and assist mothers for breastfeeding.

• Performs suctioning through the nose, mouth and ET tube.

• Administers oxygen therapy per incubator, mask or cannula.

• Uses pulse oximetry for oxygen saturation monitoring and cardiac rate monitoring.

• Operates intravenous syringe pumps for IV therapy.

• Performs nursing care to newborn in the ventilator, inside incubators and on CPAP.

• Assist Pediatricians in special procedures such as IV insertions, umbilical cannulation, ROP screening, lumbar puncture, TPN/hyperalimination, chest tube insertion, etc.

• Assist technicians during x-ray procedures, med techs during blood extractions on patients.

• Documents patient care services by charting in patient's chart completely and accurately.

• Prepares newborn for circumcision and other special procedures that can be done at NICU unit by securing consent form from the parents.

• Takes care of newborn post-operatively.

• Assists junior consultants or pediatricians during resuscitation of infants by doing CPR, preparation and administration of emergency drugs, ambu -bagging, suctioning and documenting of events.

• Takes care of infants undergoing intensive and regular phototherapy.

• Facilitates laboratory requisitions; ensures accurate laboratory request and proper labeling of specimen.

• Encodes in the hospital computer system all procedures, instruments and supplies taken, ensures accuracy and completeness of data.

• Documents accurately in the nursing progress notes the vital signs, medications, treatments, IV therapy, intake and output and any other relevant information regarding the newborn condition.

• Transfers newborn with junior consultants to other hospitals via ambulance.

• Gives post mortem care as necessary.

• Carries-out pediatricians' orders and transcribes them in the Kardex.

• Knows the location and contents of the emergency cart and assumes special assignments such as checking of emergency cart, refrigerator temperature and condition of all equipment.

• Practices good professional relationships with parents and other family members, doctors, co-workers, and other hospital staff.

• Observes and maintains cleanliness of the unit.

• Observes hospital infection control policies and procedures in the unit and hospital.

• Provide and explains the discharge instructions to parents.

• Assist in the orientation and evaluation of new staff and trainees.

JOB DESCRIPTION

• Directing the admission of newborn, discharge and general flow of newborn patients, and assigning nurses and midwives to patients.

• Makes patient assignment and classify patients based on 3 levels. Level 1: care of healthy and well newborns. Level II: care of newborn under antibiotic and IV therapy, oxygen therapy, and under phototherapy. Level III: care of premature and ill babies under intensive care.

• Perform clerical duties such as processing documents and making follow ups of diagnostics results and referrals.

• Makes nursing rounds and endorsements together with the staff, clarifies and verifies nursing procedures to be done for the receiving shift.

• Carries doctors' order promptly and makes sure that it is well implemented.

• Attends to newborn deliveries at Delivery Room and Operating Theatre and initiate and perform resuscitation procedures on critically ill neonates in the absence of Pediatric Junior consultants.

• Ensure that the care delivery is within the guidelines and policy of the NICU unit.

• Provides direct nursing care to the patients, and to supervise the day-to-day nursing activities of the nursing staff, midwives and auxiliaries.

• Provides for accurate descriptive records of the medical treatment carried by the staff nurses and the nursing care given to the newborns.

• Confers with the doctors on newborns' progress and reporting special circumstances.

• Makes rounds with physicians when Junior Consultants is not around.

• Ensures that an adequate supply of NICU stocks medications, supplies and equipment is on hand to meet the nursing needs of the newborns. Reports the needs and repairs to the Head Nurse.

• Encourages attending physicians to review treatment plans, record and sign their orders, progress notes, etc., in accordance with established policies.

• Coordinates with other departments regarding the newborns needs.

• Completes and files required record-keeping forms/charts upon the newborns admission, discharge and/or transfer.

• Ensures that all staff knows and understand department policies and procedures concerning CPR, fire and earthquake disaster and baby tagging.

• Ensures that all staff follows correct infection control procedures at all times and report any problems.

• Facilitate and conducts newborn and hearing screening for all newborns delivered at UCMED.

• Provides orientation, guidance and direction, training to new staff and students, and participates in clinical teaching and in ongoing clinical education for all staff.

• Anticipates, prevents and resolves any problem arising, reports untoward incidents/accidents, staff sickness and absences to the Head Nurse.

• Promotes patients' education such as Breastfeeding importance and policy, Rooming-In promotion, Newborn and Hearing screening importance.

• Performs any other tasks within her competence as directed by Head Nurse or Chief Nurse.
