(ICU, In-Patient, office, lounge) Clean Discharged rooms. Collect trash, soiled linen, & Bio-hazardous materials using proper safety procedures.

Conducts new client Intakes & HMIS assessments. Assist each client with developing a 6 month case plan within 60 days of established residency. Exit participants from program as needed.

Perform regular weekly visits to all assigned housing sites & complete weekly case notes on all clients, documenting all inter actions & incidents.

Respond to emergencies & perform crisis intervention as needed. Organize house meetings to discuss any conflicts or issues in the house or between roommates. Transport & accompany clients to appointments. Act as a liaison & advocate on clients behalf.
