•Position Title : Client care Coordinator

•Intake,Athorization Department

Position Summary :The Client Care Coordinator is responsible for assigning patient referrals as directed to agency para-professional and professional staff and under the direct supervision of the DPS/RN designee. Responsible for maintaining communication with agency personnel, HHA/PCA and contracts agencies for the daily operations of the agency . Provides basic case management services .

Essential Functions :

•Responsible for planing, organizing and providing clients with appropriate Home Health Aide services in a timely and efficient manner .

•Responsible for coordination of daily Home Health Aide assignments and coverage for sick calls/ no call / no shows .

•Responsible for on call coverage of client services to ensure Home Health Aide placement and services .

•Participate in Department of Heath and Joint Commission surveys ; and the organization 's Continuous Quality Improvements process .

•Responsible for completing Incident /Accident Reports with Timely submission to Director of Patient Services or designee.
