• Provide direct case management services to a minimum of 15 clients (typical caseload may be as high as 60)
• Partake in the team-based assessment and contribute through the collection of data and creation of documents to assist the LHPA in additional treatment goals and documentation
• Maintain the clinical record which includes completion of all required paperwork and on-going progress notes for each client contact; Prepare and maintain required paperwork as outlined in the Federal, State, and/or City & County DPH guidelines (i.e. Documentation Manual); capture all billing opportunities to ensure meeting the minimum number of minutes per week as required by the Programs’ annual contract.
• Liaison with other treatment providers, family members, others involved in the client’s care and community functioning. Engage in case conference at Westside or out in the community in efforts to benefit the client.
• Provide crisis intervention as needed which may include community outreach and hospitalization of the client. Mandatory reporting to CPS in addition to the completion of filling out a Quality of Care report for DPH.
• Participate in weekly mandatory meetings: team meetings, productivity meetings, and weekly supervision; as well as participate in professional development training as well as all required safety trainings and other required annual DPH and/or Westside trainings throughout the year.
• Participate in a community events that may fall during evenings, weekends, and holidays
