Revive Inc. Recovery Facility - Short Term Residential Case Manager
Manage day to day resident needs such as new patient orientation. Gather client medical information, and any prior recovery records. Input and update all client info. on computer software. Sign clients up for medical insurance and/or Medicaid based on eligibility. Follow up with medical health insurance companies for billing and authorizations and FMLA/short term disability needs as well. Provide transportation and accompany clients to offsite medical appointments. Attend weekly Case Manager meetings, staffing and monthly CEU (continuing education courses). Provide outreach services to local community to educate about the facility and its program. Mentor clients in relearning life skills, daily living skills and transitioning to independent living upon completion of short term residential program.

Grand Island Public Schools - Homeless and Transition Coordinator
Facilitate and coordinate welcome packets for homeless children and their families. Provide homeless families with access to resources to ensure their children are successful in school. Attend annual training, and coordinate annual programs like Food for Thought, Christmas Baskets and Thanksgiving Baskets. Solicit and coordinate pick-up and delivery of donations for EIT (Families in Transition). Process paperwork for free and reduced lunches, provide case management to families and interact/communicate with school Social Workers.

EDN/Head Start Services Coordinator
Represent children from birth to age 3 who have a diagnosed developmental delay that qualify for services through a resident school district program. Responsibilities included evaluation of child to determine if they met the qualifications for services, processed the paperwork to enroll them in the program at no cost to their families. Program provided in home services as well as monthly visits and twice a year holding IFSP/IEPS.
