Objective

My experience includes 15+ years in social services working in community outreach, drug & alcohol counseling, at risk children, program coordination and management with advanced training in chronic disease self- management, diabetes self-management, and fitness training for adults primarily with balance and fall risk issues. In addition to my background in social work and sales, I also have a background in patient care, formerly a certified nurse assistant during the early 1990’s and a student nurse working in the home health care field with several years of experience doing home visits. As program coordinator, I was delegated to bring awareness to two rural counties in North Florida, Bradford and Union County, about the importance of disease prevention through healthier lifestyle initiatives and educational programs offered to the people of the community. Special emphasis was placed on populations disproportionately affected by chronic disease and the least likely to receive the necessary interventions because of lack of transportation and lack of abundance of resources in these rural areas.
The primary components of these healthy initiatives encouraged disease prevention with a primary focus on diabetes, heart disease, stroke, obesity and tobacco use. I encouraged healthy living through offering education on tobacco cessation, involvement in physical fitness programs and community events and nutrition education.
