Daily goals submitted by clients were completed. Hand over hand assistance provided, reading and interacting with clients via games and other activities. I work with clients with developmental disabilities, and people identified by the client, such as family, to develop a written individualized plan of services. As a support coordinator I monitor the client with the appropriate services, and continually follows-up to ensure the client is receiving the best care. I also provide assistance with access to entitlements, such as Medicaid. Community Access provides Home and Community-Based Services to individuals who benefit from less structured approaches to service delivery. UCP services are provided to individuals who meet eligibility requirements and have been authorized by the City to receive the services. A broad array of options are available through this program. Supports may be offered in a variety of ways such as through supported home-living in which services are provided in one's own family home. Residential support services are offered through community homes in which staff are required to pick up client in a van and proceed to community outings such as, malls, museums, restaurants, movies etc...
