Responsible for:

Coordinating support services to assist participants with accessing community resources to

strengthen independent living skills.

Monitor and review case file documentation and monthly reports.

Providing linkage and referral to clients self-referred and referred by DCFS.

Coordinating support services for clients to address identified needs.

Facilitate groups and corresponding documentation.

Engage in monthly outreach events.

Primary Duties:

Assist in the monthly review of case files and documentation. Provide appropriate feedback to

staff and follow up on necessary corrections.

Facilitate weekly group supervision, or more frequently for higher-needs families, and complete

Attends and participates in program, agency staff meetings, and related community meetings as

Collaborate and coordinate with contractors, SHIELDS staff, and program partners and components to ensure continuity of service provision.

Partner close with other Homeless Service providing Agencies.

Primary Duties

Program Manager participates in activities related to personnel, such as, recruiting, interviewing, evaluating and conflict resolution.

Completes all required forms and reports as required by the funding agency.

Develops relationships, and provides linkages between housing programs, other SHIELDS programs, and the community, and outside agencies; represents the SHIELDS program at various outside functions and meetings.

Coordinates with lead agencies regarding program design, outcomes monitoring and reporting, and QA/QI.

Develop housing resource networks with shelters, transitional housing, project-based housing, and negotiate permanent placement.

Complete quarterly housing service plans and other documentation per funding requirements.

Assist residents with achieving independent living skills.

Mediate conflicts between tenants and adhere to SHIELDS protocol for conflict resolution and provide input in the formulation of an action plan to rectify concerns.
