OBJECTIVE:
I am actively pursuing a challenging role in case management that will build upon the diverse experiences I have accumulated in my previous positions. I aspire to become an alcohol and substance abuse counselor; however, I am currently not state-certified and still need to complete my clinical hours. I am also a State-certified peer support specialist, a credential often vital in the social services sector.
My background in case management enables me to leverage the education and knowledge I have gained through my studies alongside the skills and experiences I have developed over the years. These strengths equip me to deliver high-quality case management services effectively.

1.Conduct client intake and assessments and identify, develop, and meet goal-oriented outcomes that ensure clients' success.
2.Ensure that clients know and understand why informed consent is essential in all aspects of case management to secure a positive outcome for their goals.
3.Manage a caseload and coordinate all case management services, including developing an MIRP and completing the behavior health assessments. Determine acuity and develop a crisis plan.
4.Engage with clients 1-4x month and meet the client’s outcomes by determining the objectives needed to reach the client’s recovery goals and identifying services that may be applied.
5.Make collaborative outreach attempts, including transporting clients to other involved or needed providers to coordinate members' care as necessary.
6.Participate in crisis intervention services, assess needs, and develop and coordinate written crisis plans when indicated in coordination with the client’s care team.

**Job Title: State Certified Peer Support Specialist/Case Manager**
Overview:
As a Peer Support Specialist, my primary role involves providing comprehensive support and guidance to clients as they work towards achieving independence or transitioning into clean and sober housing. My responsibilities encompass various activities to enhance each individual’s journey to reintegrate into society.
Key Duties:
Progress Monitoring: I meticulously update weekly progress reports and conduct monthly assessments to track each client’s transition progress. This allows for tailored support and timely interventions as needed.
Resource Utilization: I assist clients in identifying and utilizing available local resources to aid in their reintegration into society. This includes connecting them with community services, employment opportunities, and supportive housing options.
Group Programming Facilitation: I lead various structured group sessions to develop essential life skills. These include but are not limited to anger management, relapse prevention strategies, peer-to-peer support interactions, and conflict resolution techniques. The aim is to empower clients with the tools they need to navigate life’s challenges effectively.
Client Advocacy: I advocate for my clients, actively seeking and securing resources that align with their needs and best interests. This advocacy is crucial in helping them achieve their personal goals and fostering a sense of agency in their recovery journey.
Through these efforts, I strive to create a supportive environment that promotes healing, personal growth, and successful reintegration into the community.
