Dear Godwin Ikechukwu Adom:

I am pleased to tell you that we have approved your application for income support from the Ontario Disability Support Program (ODSP).

This letter will help you understand:

● how much income support you will receive

● your start date for ODSP

● other ODSP benefits that are available to you

● your reporting responsibilities

● working while on ODSP

About your ODSP income support

You qualify for income support as of November 1, 2024. We will pay you income support starting from this date. We have included a chart with this letter to show you how we calculated your income support. As per the chart below, you will receive a retroactive payment of $1,185.00 from ODSP.

You will receive your monthly ODSP income support by direct bank deposit at the end of each month. You will receive $399.00 from ODSP at the end of this month. Important information about your ODSP payment

● You received $387.00 from Ontario Works for this month. That is why you will receive less money from ODSP this month.

Remember: Because of the Ontario Works payment at the beginning of the month, you will receive less money from ODSP at the end of the month. Please keep this in mind as you budget for your expenses.

How we decided your start date for ODSP

Your start date is November 1, 2024. Because you were receiving Ontario Works when you applied for ODSP, your start date for ODSP is the first day of the month after we received your complete application.

You cannot appeal our decision that your start date is November 1, 2024. This is because the Social Benefits Tribunal is not allowed to hear appeals about this decision. However, you can appeal if you do not agree with our decision that your application was complete on October 17, 2024.

About your other benefits from ODSP

You can use your Ontario health card to get some of your benefits:

● prescription drugs

● dental care

● routine eye exams

Each family member will use their own health card. Just show your card at the pharmacy, dental or optometrist office so they can check if you qualify. If you don’t have an Ontario health card, you can show: Page 1 of 6

● another piece of identification

● your monthly statement of assistance

Your reporting responsibilities

You must contact us to tell us about any changes to your situation, including changes to:

• your income

• where you live

• how much you pay for housing

• if someone leaves or joins your household

• the amount of assets you have.

If you work, you must tell us each month about any earnings that you receive. You can report your earnings in one of the following ways: 1) Send us the Employment/Training Income Report form. You can also include child care or disability-related work expenses on this form. 2) Report by telephone through the Interactive Voice Response at 1-800-***-****. 3) If you have a MyBenefits account, you can report your earnings online at Ontario.ca/MyBenefits.

You must report any earnings received from the first day of the month and the last day of the month by the 7th of the following month.

For example: You receive $250 between September 1 and September 30. Report your earnings by October 7.

Keep your pay stubs and any receipts for expenses. We may ask you to provide them when we review your case.

Working while receiving ODSP income support

You can work while receiving income support. If you want to work, you may be able to get help to find and keep a job – even if you have never worked before or have been out of work for some time.

In addition, ODSP may be able to help you to:

● cover some of your costs of working, and

● get the supports that you need.

If you do not agree with our decision

If you do not agree with a decision in this letter, you can ask us to review it. This is called an internal review.

You must ask for an internal review in writing by February 25, 2025. If you miss this deadline, you can still ask for an internal review, but you will need to explain why you were late. If you were late for a reason beyond your control, we may be able to do an internal review even if the deadline has passed.

If we do not change the decision after the internal review, you may be able to appeal to the Social Benefits Tribunal. Most decisions can be appealed to the Social Benefits Tribunal. If you do not ask for an internal review, you will not be able to appeal and this decision will be final.

We have included information on how to ask for an internal review and appeal a decision with this letter.

If you have any questions or need more information If you would like to talk to someone about this letter or your case, please contact the ODSP Office by calling 905−897−3100.

You can find more information about ODSP on the ministry’s website at www.ontario.ca/community.

HOW WE CALCULATED YOUR ODSP INCOME SUPPORT

This chart shows how we calculated your ODSP income support for each month from your start date until now.

Here’s how it works:

We add up your ODSP income support maximum for each month. (This is the maximum amount of ODSP income support that you are allowed to receive based on your family size and housing costs).

Then we:

• subtract any money owed as an Overpayment for the whole period (including any money you paid back while receiving Ontario Works)

• add the total amount under Additions (these are any new benefits from ODSP)

• subtract the total amount under Deductions (this includes your money from Ontario Works and any other income)*.

After we add and subtract these amounts, we have the total amount of ODSP income support that we owe you from your start date until now. This is your Total ODSP Payment. Start Date: November 1, 2024

THE INTERNAL REVIEW PROCESS

If you do not agree with a decision about your Ontario Disability Support Program (ODSP) case, you can ask for a review of our decision. This is called an internal review. An internal review is done by staff at your local ODSP office. The person who does the review will not be the same person who made the decision you do not agree with. You have to ask for an internal review before you can appeal to the Social Benefits Tribunal.

What you need to do

To make your request, you must write a letter or fill out a Request for Internal Review form. You can get this form from your local ODSP office or online:

(www.ontario.ca/page/internal-reviews-and-appeals-social-assistance). If you write a letter, you should:

● say “I am requesting an internal review”

● explain why you do not agree with the decision that was made (optional)

● attach any additional information that you think may help in the review

● include your full name and your member ID (this number is at the top of the decision letter)

● include your signature and the date.

Please mail, fax or hand deliver your internal review request to your local ODSP office within 30 days of receiving the decision letter.

Things to keep in mind

● If you cannot send us your request within 30 days because of reasons beyond your control, we may still do an internal review. Just tell us in your request why you need more time.

● Keep a copy of your request – you will need it later if you appeal the decision to the Social Benefits Tribunal.

● If you do not request an internal review or you withdraw your request, the decision will be final and you will not be able to appeal to the Social Benefits Tribunal. Results of the internal review

We should give you the results of the internal review within 30 days of receiving your written request. If you do not receive the results of your review within 30 days, you can appeal to the Social Benefits Tribunal.

THE SOCIAL BENEFITS TRIBUNAL APPEAL PROCESS

You have 30 days to appeal to the Social Benefits Tribunal. Here is how it works:

● If you receive a letter telling you the results of the internal review, you have 30 days to appeal from the day you receive the letter.

● If you do not receive a letter telling you the results of the internal review within 30 days of your request being received, you have 30 days to appeal once the 30 days have passed.

What you need to do

To make an appeal, you need to fill out an Appeal Form and mail, fax or hand deliver it to the Social Benefits Tribunal. You can get an Appeal Form from your local ODSP office, from the Social Benefits Tribunal or from their website: Social Benefits Tribunal
