Health Promotion focal point person. Designing of SBCC activities / plans such as Drama and community meetings, IEC key messages for all health facility activities in the community. And identification of communities that need the services.

Overall supervisor for all community based volunteers (CBVs) in all departments at the health facility. Selection and allocation of each CBV is my role at the health facility. This includes monitoring and evaluation of their reports, coordination of all the ministry of health community structures such as CBVs, NHCs, and HCC. And ensure that they are active. And updating them on new developments such as outbreaks, campaigns from all departments.

Design SBCC strategies to help implement public heath activities in the community to create demand and microplanning. Such as immunization programs for under 5 children (NOPV and BOPV (polio) campaigns, late ANC bookings, OCV (cholera vaccine) campaign, measles).

Maintenance and monitoring of cold chain of all vaccines at the health facility

Adolescent Health focal point person for 3 years and planning for their activities.

Interpret data collected and preset it to NGOs and higher offices for decision making in case we have a rooming outbreak.

Present findings in form of graphs, curves, charts, tables etc. to the health facility staff, NGOs and community leaders.

Conduct disease mapping and develop risk maps for different diseases. Contact tracing Conduct active disease surveillance and sample collection from the suspects. (TB, new HIV,AFP, measles, cholera, Dysentery).

To orient all new and old staff in Disease surveillance and Epidemiology in health facilities.

Infection Prevention Focal Point Person, to Conduct trainings in infection prevention and control (protocols) to all health staff and support staff and CBVs working the health the facility (DBS and HIV testing services ), to ensure patient and health care provider safety during service provision.

Conduct weekly disease surveillance, monthly and quarterly review meetings with all health facility staff and community health structures.

Facilitate the collection of samples from disease suspects, storage and transportation to the appropriate lab for analysis and confirmation and follow up results. Then disseminate the findings to all the health facility staff.

To development of a Risk Communication and Community Engagement (RCCE) Action Plan for all health problems such as cholera outbreak which was recent, high number of STI infections in Adolescents, ANC,TB,ART,NACs, FP and GBV in the community. Selection and training CBVs.

TB focal point person for 3 years. Staff screening for TB. Ensure all patients are cured and defaulter tracing is done and patient tracking is done. Co infection management and provide linkage of HIV clients to treament and defaulter tracing.

Conduct nutritional assessment (NACs) in under 5 children and Adults at the clinic and disperse nutritional food supplements to eligible clients. Such HEPS and RUTF.

Coordination of community nutritional triggering for under 5 children and their referral to the health facility if found stunted, underweight or obese. Health education on food and good nutrition for under five children.

Infection Prevention Focal Point Person, ensuring all HIV negative clients at risk have access to prevention services such as PREP and PEP. And ensure documentation for staff is done in order to access risk.

Coordinate and organize outreach services with MCH to communities and schools for mobilization and HPV vaccinations and enrollment.

Mapping of NGOs, stakeholders, gatekeepers and other leaders in the community and ensuring that they are engaged for them to help us with resource support/mobilization and Advocacy.

Writing of narrative and other comprehensive Reports following the completion of an activity.

Planning and budgeting for disease surveillance and epidemiology research and other activities.

Monitoring and evaluation of the implementation and impact of active disease surveillance at the health facility monthly with all health staff.
