Create, plan, and direct the hospice care team utilizing nursing processes of assessment, interventions, implementation and evaluation. With compassion, ethics, professionalism, and the highest standards of care, manage the care of 12-20 patients on hospice who reside in assisted living facilities, nursing homes, and private residences.
