MAFB Short Term Pass Request

Instructions

•This request is specifically for use by personal assigned to McConnell AFB

•Fill out all areas completely by tabbing through each of the blocks. All fields are mandatory for each visitor. A total of (5) guests can be sponsored per day.

•Incomplete requests will not be processed. Please allow 24 hours prior to your visitor's arrival for processing. Same-day requests will not be processed.

•Inform your visitor(s) to have their current driver’s license, available for review.

•Contractors and or vendors using company vehicles are required to be inspected during the hours of 0600 to 1400 weekdays and must use the WEST/CVI gate. Those commercial vehicles going into Mayroad Military housing will be inspected/processed at the Visitor Center. This request is limited to a maximum of ten (10) days.

•As a reminder there are no weapons, fireworks or explosives of any kind allowed on the installation. For Vehicle operators, there is no talking or texting unless on a hands free device.

•Once you have completed the request, save the document, attach it to an email and send it encrypted to the appropriate office below. Your email must be from a .”mil” address.

Note: A member can only sponsor a maximum of five persons a day. It is prohibited to sponsor Special Events or non U.S. Nationals on this request.

This pass request is for sponsoring up to (5) visitors per day by a military or authorized DoD civilian. All information is mandatory. Incomplete requests will not be processed. The visitor(s) must present their current driver's license . The sponsor, need not be present once processed. Contractors/vendors, during normal duty hours, must use the West Gate Commercial Vehicle Inspection. Commercial Vehicles going into Military Family Housing will inspected/processed at the Visitor Center. It is strictly prohibited to use this pass to sponsor Special Events or non U.S. nationals. The sponsor is wholly responsible for their guests while on the installation.
