Developed Caregiver Coaching program for new start-up caregiving app.

Managed caseload of approximately 25 - 30 working caregivers, caring for loved ones with cancer, dementia, profound disabilities, autism, behavioral health issues and others. Provided caregiving related education, self-care health and wellness coaching, resources for both caregiver and caregivee.

The pilot program showed exceptional success in reducing ER and inpatient stay utilization by 30% and 30-day inpatient re-admission by 70%.

Analyzed well-patient visit metrics, showing an improvement in all available measures, such as dental visits, lead screening and engagement with primary care.

Provided leadership and management to 30 – 40 staff and hundreds of volunteers in multiple departments during a time of growth, reorganization, and transformation for Whitman-Walker Health.
