Assists in the collection of pertinent data to be utilized by the hospice provider and/or delegating RN in the assessment of the patient's physical and mental changes including age specific and cultural needs. Accurately takes, records, and reports vital signs, pain rating, and I & O (including drains). Performs specific nursing interventions as delegated by RN per protocol and documents response. Delegates tasks and duties in compliance with policy and in a manner that results in efficient and effective patient care. Participates in change of shift bedside reporting. Safely transports, transfers and ambulates patients using appropriate body mechanics and equipment. Performs purposeful hourly patient rounding. Implements and adheres to Fall Prevention Program. Demonstrates ability to function effectively in clinical emergencies following all established protocols. Minimize infections by implementing isolation precautions when indicated, being compliant with isolation procedures, and maintaining cleanliness of the facility. Enhances patient and family experience by: courteous and compassionate interactions with patient and families; compliance with professional dress code per policy; maintaining a clean and orderly patient room; and, placing personal items within safe and convenient reach of patient. Communicates effectively with co-workers, physicians and other healthcare workers. Promptly responds to all patient needs and calls in a timely manner to ensure patient safety and maximize the patient experience. Respects patient privacy by always knocking before entering room and providing dignity and respect when performing nursing interventions and assisting with ADL's. Participates in patient and family education and provides companionship and emotionally supportive care to patients, families and/or caregivers.
