Accomplishments

●I was the Coordinator and eventually the Principal Investigator of the DANTE trial, the first randomized trial to be launched in the world that compared lung cancer screening with spiral CT vs. no intervention in high-risk subjects. In this study 2452 subjects were recruited and followed for a median of 8.35 years. This effort led to the detection of 104 patients (8.23%) lung cancer cases in the screening arm (66 by CT), 47 of whom (3.71%) had stage I disease. I have been since a member of an international group that promotes the implementation of lung cancer screening. (Infante M, et al; DANTE Study Group. ●A collateral benefit of the the DANTE study was that it gave an impulse to the development and refinement of minimally invasive lung cancer surgery, that is nowadays the standard of care for early-stage lung cancer but was not so in the early 2000’s when the study was launched. I was eventually elected a member of the board of directors of the Italian VATS Group prospective registry and remained in charge until 2018. (for example please see: Infante MV, et al . ●The development of lung sparing lung resection techniques (i.e. segmentectomy by VATS) is one of my achievements as I have successfully completed more than 100 VATS pulmonary segmentectomies with zero mortality and less than 10% morbidity, and developed technical tips and tricks to accomplish the procedure that have been presented at several national meetings since 2017

●Another accomplishment was the development and diffusion of lung nodule management protocols that have improved the outcomes for patients and are in use in the Hospitals where I have worked. ●I have improved technical procedures for the management of early-stage and locally advanced mediastinal tumors (for example please see: Infante M, et al. ●I have perfected the surgical technique for the Nuss repair of pectus excavatum with approximately 300 cases successfully completed, two-thirds of whom are adults and with a low 1.8% bar displacement rate (Infante M, et al. Direct a wide range of procedures inclusive of state-of-the-art comprehensive management of all thoracic conditions. Collaborate with multiple departments such as Spinal Surgery, Cardiac, and Head and Neck. Perform surgery for malignant pleural mesothelioma, pleural empyema, lung abscess, chest wall stabilization after major trauma in addition to esophagectomy-gastrectomy cases on upper GI patients. Carried out or assisted some 15 robotic major lung resections.

Coordinate the multidisciplinary thoracic oncology team on a weekly basis to discuss complex cases, including from neighbouring hospitals. On average, 10 to 15 cases are presented and 15 to 20 specialists and trainees attend the on-line meeting. This Multidisciplinary on-line group is considered the best-performing MDT in the University Hospital of Verona.

Oversaw the thymoma and myasthenia gravis multidisciplinary team until 2023.

●Completed approximately 1,000 major lung resections for cancer and/or MDR mycobacterial infections, as well as 250 mediastinal procedures and 70 VATS thymectomy for thymoma and/or myasthenia gravis.

●Managed a variety of chest-wall conditions and carried out resections for sarcoma or lung cancer invading the chest-wall, chronic infection, stabilization for trauma, and over 400 chest wall deformity cases

●Managed invasive lung cancer staging (videomediastinoscopy or mediastinotomy, EBUS),

●Effectively reduced the mean hospital stay after major lung resection from 9 days to 5.4 days on average and maintained perioperative mortality below 0.5% over the past few years.

Supervised the ward in the 720-bed tertiary referral centre. Conducted weekly chest surgery clinics, as well as clinical audits. Provided executive-level consultancy within the Emergency Department. Facilitated weekly cross-functional team meetings. Served as a member of the panel focused on the issuance and review of internal management guidelines for lung cancer, mesothelioma, and thymoma.

●Initiated an institutional pectus surgery program, VATS-lobectomy and VATS-thymectomy program.

●Promoted lung-sparing surgery and pleurectomy / decortication for mesothelioma which is now the standard approach for the department.

●Introduced improvements in the operative and postoperative care of esophageal cancer patients with enhanced recovery after surgery by effecting early administration of liquids and a soft diet per mouth resulting in a postoperative stay of less than ten days.
