● Functioned as a Team Leader of ICU.

● Assists in maintaining high standards of patient care in the unit/department.

● Neurology cases as cerebrovascular accident (CVA), seizure disorders, epilepsy, Guillain Barre Syndrome (GBS), Myasthenia Gravis (MG), Amyotrophic Lateral Sclerosis (ALS), vasculitis which needs plasmapheresis or immunoglobulin infusions.

● Psychiatric cases as depression, bipolar disorder, schizophrenia, drug abused.

● EENT cases as conjunctivitis, keratitis, otitis media, vertigo.

● Cardiology cases such as myocardial infarction, hypertension, acute coronary syndrome, patients with pacemaker, post coronary artery bypass graft, deep vein thrombosis, stenosis, atrial fibrillation on heparin infusion; titrating inotropic infusion (milrinone, dobutamine) and vasopressors (adrenaline, noradrenaline and vasopressin).

● Respiratory cases as Acute Respiratory Disease Syndrome (ARDS) requiring high PEEP, proning position and mechanical ventilation, prolonged ventilation requiring tracheostomy, Chronic Obstructive Pulmonary Disease (COPD), Obstructive Sleep Apnea (OSA) requiring BIPAP management, Pulmonary Embolism (PE), pneumothorax, hemothorax on chest tube management, and pleural effusion on pleural fluid drain, influenza, pulmonary tuberculosis that needs mechanical ventilation, negative pressure room and anti-TB drugs.

● Gastroenterology cases as Upper and Lower GI bleed that needs massive blood transfusion due to severe blood loss, colonoscopy or endoscopy or embolization, stomach ulcers, bowel perforation or ischemia, Chron’s disease.

● Genitourinary cases as Kidney Diseases requiring Continuous Renal Replacement Therapy (CRRT) or Peritoneal Dialysis (PD), metabolic acidosis and hyperkalemia requiring dialysis. CRRT machine is under the scope of ICU staff nurses’ practice; certification was obtained first prior to CRRT priming and management.

● Endocrine cases as hyperparathyroidism, hypothyroidism, diabetic ketoacidosis, diabetes mellitus, Addison’s disease.

This kind of patient are kept fully sedated initially with fentanyl and midazolam infusion most of the time to give full mechanical ventilator support.

Full time position: 12 hours duty per shift/an average of 60 hours per week Responsibilities:

● First task is to receive endorsement of emergency cart (emergency medications and emergency equipment). Check it properly. Ensure that emergency drugs, equipment or devices are properly replaced and stored after use.

● Endorse patient/s to morning or evening shift co-staff nurses for continuous intensive care.

● Give skilled intensive nursing care in patients according to their individual needs; utilizing the nursing processor scientific problem-solving approach.

● Close monitoring of ICU patients, taking of vital signs (blood pressure, cardiac rate, temperature, respiratory rate) every hour manually and with the use of cardiac monitor with probe (or pulse oximeter); direct observation-keen observant on the neurological status of the patient every 1 hour (using Glasgow Coma Scale and assessment of pupil size and reaction to light) and report immediately to resident physician on duty any abnormal findings for necessary immediate actions.

● Performs physical examination – identifies relevant signs and symptoms that will contribute in establishing diagnosis.

● Alertness in rendering cardio-pulmonary resuscitation especially in emergency situation.

● Give medications (oral, subcutaneous like insulin, anti-thrombolytic pre-filled syringe and epoetin beta, intravenous, intramuscular and through gastric tubes.)

● Observed proper disposal of syringes, needles, IV catheter, IV bottles, IV tubings or other medical tubings like suction catheter, foley catheter, nasogastric tube, nebulization kit, corrugated tubing’s, t-piece, APN bottle; then separate infectious to non-infectious.

● Maintain good communicating skills with patients and patients’ relatives.

● Provide and secure consent to procedures or operation that has been ordered by attending physician.

● Conduct pulmonary assessments to identify abnormal respiratory patterns or breathing sounds that indicate problems especially patients on mechanical ventilator.

● Collect specimens for laboratory tests like Urinalysis, Fecalysis, Sputum Gram Stain, Culture & Sensitivity, Endotracheal Tube Aspirate, Sputum & Acid-Fast Bacilli Smear, Random Blood Sugar and Arterial Blood Gas Exam.

● Evaluate patients' vital signs and laboratory data to determine emergency intervention needs, then relayed to attending physicians and resident on duty.

● Monitor accurately patients' fluid intake and output to detect emerging problems such as fluid and electrolyte imbalances, then referred to attending physicians immediately for any abnormalities.

● Administer blood and blood products, monitoring patients for signs and symptoms related to transfusion reactions.

● Compile and analyze data obtained from monitoring or diagnostic tests. Knowledgeable of its normal values and report to resident physician abnormalities noted. • Maintain patients’ safety.

● Securing written consent in all invasive procedures.

● Turning of bed ridden patients every two hours to maintain skin integrity and blood circulation and to avoid bedsore especially to cases like CVA patients with weakness.

● Advocate for patients' and families' needs, or provide emotional support for patients and their families.

● Prioritize nursing care for assigned critically ill patients based on assessment data and identified needs.

● Document patients' medical histories and assessment findings. • Provide post-mortem care

● Identify malfunctioning equipment or devices.
