** Note **

Reason for non-employment In August of 2018, I was performing my duties as a Home Health Nurse driving from a client’s home to another.. I was involved in a MVA where another driver ran a red light and broad-sided me, causing a fractured right and left shoulder, a fractured right scapula, and fractured left eye. I went thru 4 surgeries and returned to work. I continued to have discomfort in my right shoulder, so in November of 2021, I decided to relocate to Southern Texas, hoping the warmer weather would help relieve the pain. I arrived in McAllen Texas in March 0f 2022, but I continued to have discomfort. I was referred to an orthopedic Surgeon, and he recommended surgery as there were pins protruding out. In November of 2022 I had a total shoulder replacement, went thru 6 months of physical therapy, and I have been cleared to return to work.
