Provide effective telephonic lifestyle coaching, health education and disease management to employees

and patients enrolled in the high- risk care management program.

Empower patients to better understand their illness and self-manage their condition.

Provide direct telephonic clinical education and case management to patients through one-on-one teaching to address gaps in care and disease management.

Complete initial health assessments for patients enrolling into the high-risk Health Disease Management and

Lifestyle Management phone coaching programs.

YMCA After School Leader

Led, developed, and implemented nutrition education, physical activity, and cooking demos for high schoolers.
