Responsible for Intake process, including establishing and maintaining positive relationships with clients,

referral sources, responding to customer requests and concerns, and managing the insurance verification.

& Authorization process. Completes all patient referral and intake. Enters referrals, contacts families to

explain Hospice philosophy. Medicare/ Medicaid. Ensures compliance with all state, federal, and CHAP

requirement. Gathers and collects all reports for referrals, Tucson, Palliative and Outreach Programs.

Completes all monthly reporting for visits with our staff for IDG. Prints all IDG notes, medication list

for staff. Updates weekly frequencies. Orders incontinence supplies for drop ship to be done weekly.

Volunteer Notes and coordinates with facilities/ private home with patient scheduling. DME ordering,

updating and picking up schedule daily. Reports on missing equipment for follow up. Assists with

Bereavement cards, letters, and care notes monthly. Provides leadership in strategic planning including

completed for daily visits and QI reviewed for billing. Receive faxed/referrals from Providers, Hospitals

and Rehab Facilities. Coordinating with PCP to have the required face to face if the patient was Medicare and following up within 5 days for start of Service. Data entry of all patient information including insurance verification. Follow plan of care for scheduling disciplines. Reporting and ensuring rectifications are completed in a timely manner. QI audits performed on RN documentation for start of care. Communicating with multiple insurance companies to verify benefits if not online. Tracked and communicated out to clients or POA'S monies owed for visits not covered by insurance.
