Providing outreach, enrollment, engagement, eligibility screenings, assessments, and planning with adults as an
embedded Health Home Care Manager within a U of R Family Practice. Collaborating with 6 Primary Care
Physicians, Nurses, and administrative staff on a daily basis. Linkage and referrals to community providers,
programs, and resources as needed to reach physical, mental, and behavioral, health goals, as well as to
overcome social and economic barriers. Engage with patients at the practice and in the community. Monthly
billing caseload of 45 patients.
Children’s Health Home Care Manager – Pathways, Inc., Rochester, NY Aug 2018 – Nov 2019
Providing outreach and engagement for youth and families that have been referred for Health
Home services. Completing screenings and assessments to develop comprehensive plans of
care and individualized crisis plans. Collaboration with numerous providers as well as
referring to identified community providers and programs as needed. Maintaining
relationships with service providers to facilitate service effectiveness to support the youth and
families goals of wellness and to meet monthly billing requirements of up to 25 youth and families.
