Worked with her nutritionist to ensure dietetic meals were meet, as the intake of sugars and carbs for diabetes, and caloric count to reduce her level of weight, as well as maintain blood sugar levels to be daily meet. Did one on one counseling with her doctor and dietician from Highland Farms Givens, to be released back home after a slight heart attack and fall. Performed INR readings for weekly counts, from blood thinner medication and reported to primary doctor. Meet with and continued until Hospice was involved and used a care plan we created to care for her thru her respite care, continuing with administering and deciding what medication for her care til she passed. 2
