As you can see from my resume up until 2014 I led a very impressive social work career. Unfortunately, in 2014 I was struck down with a medical disability from which I have fully recovered. This accounts for my absence from the workforce since that time. I am guessing that some potential employers have crossed me off the list because of this gap. I hope I am not relegated to no longer being a social worker because of this! I love, love, love, my career! AND, more importantly, my intellect and social work skills have not waned, but have been sharpened during that time. Here is what I have been able to accomplish: I have completed 144 CEU’s of the 180 CEU’s required towards the Certified Advanced Alcohol Drug Counselor certification. I have completed 33.5 of the 45 CEU’s for the required social work CEU’s. I continue to hold the ACSW conferred upon me by the NASW which is a widely recognized social work credential. It shows that I have a wide range of social work knowledge and skill and that I am able to work autonomously. I have been approved for the Certified Case Manager credential (ID#AP1352) by the American Academy of Case Management. My expectations are that I will complete both by January 2024. I only need 480 hours or 3 months to apply for re-licensure of the LMSW. I have CPR certification and have secured Social Work Professional Insurance. I am working very hard to re-enter the workforce and to make myself a viable contender for a social work position. Won’t you give me a chance? Isn’t that what we do as social workers – Give second chances?

Responsible for authorizing substance abuse services including referring clients for in-patient hospital stays, partial hospitalization stays, and outpatient services. Conducted initial psychosocial assessments and determined the appropriate level of care. Conducted mid-treatment reviews, adjustment counseling and monitoring of services. Management of overall program operations of a Partial Hospitalization Program for adults with serious and persistent mental illness. Included responsibility for clinical programming, supervision and development, human resources, staffing, financial management, transportation, dietary, facility maintenance, community relations, marketing and purchased services. Duties included training new employees; providing on-going training in casework services; reviewing work performance; planning and conducting in-service training activities for school personnel; preparing psychosocial histories; treatment plans; completing intake assessments; facilitating groups; training MSW Interns and nursing students; developing and implementing procedures, forms, and programs for the unit; writing program and employee evaluations; compiling data; analyzing and completing weekly, monthly, and bi-monthly statistical reports; participating in Utilization Management reviews; attending school related meetings and participating in school committees; writing proposals for summer program grants; maintaining and managing the budget for the unit for the summer program; facilitating the 6 week summer program; writing job descriptions and job evaluations, and representing the agency at the Board meeting. Wrote yearly grants and secured money for summer programming. This program became Michigan’s model program for all Compact programs in Michigan under my design and supervision,

New Center CMHS - Mental Health Advocate - (Assertive Community Treatment). Advocate on behalf of chronic mentally ill clients with community agencies and families. Assessed, formulated and implemented treatment plans with the goal of maintaining the client in the community and avoiding frequent hospitalizations. Provided outreach services as determined by client need. Maintained accurate statistical and clinical records. Participated in team meetings, staff development and student training.
