Motivation Letter/ Personal Statement: Fellowship Transplant Surgery Surgeons have long been the gold standard of the profession since time immemorial. It may be because they save lives, the reputation that comes with their title, or their behavior. Earning a surgical degree is one of the longest and most competitive educational paths one can choose. Surgeons must be meticulous when they take the knife; underneath their fine incisions stirs the culprit — Life!

I grew up in a joint family along with my grandparents. I had a lot of people in my house to spend time with, and I played many indoor games with my cousins. I loved solving jigsaw puzzles. The idea of finding the missing piece to put together a large picture amazed me. I always used to start with the edges and work my way towards the center. This task was very absorbing, and I would immerse myself in it to escape from the rest of the surroundings, focusing on the one job at hand, giving rise to a picture that gradually made sense. Connecting one piece with the other created a star, car, or face, and the result would make me feel so happy and like a mission accomplished. While I couldn't imagine what the final image would look like when finished, putting the puzzles together led me to believe that a new picture would emerge from the uncertainty. Solving puzzles and finding the right piece is what medicine is all about, and I became more interested in pursuing a career in medicine. After rotating through different specialties in medical school, it was in surgery that I felt similar feelings to when I was solving a jigsaw puzzle.

"In surgery, it's all about when not to cut, and always remember that once done, it can never be undone" these were the words of my professor of surgery at medical school in my final year. This statement meant that one must have clear judgment and be precise before advising a surgical procedure to a patient. Subsequently, I became more interested in learning more about the field of surgery.

My liking of surgery increased when I was involved in managing a patient with bowel obstruction, and the plan was not to operate. We used the famous drip and suck protocol to manage. Understanding the choice of fluids to use, knowledge of electrolytes management, constant observation of vitals, and waiting for the patient to feel better within a timeframe gave me a sense of satisfaction. I was excited to know that the patient had eventually improved. This aspect of surgery that incorporates critical care, radiology, and clinical judgment made me interested in pursuing further training in general surgery in India and the United Kingdom. My experience in general surgery at a tertiary care hospital made me realize that surgery includes a diverse patient population and varied clinical presentations. I was involved in patient care and improved my skills by assisting and performing in the operating theatre. During my training, I learned that no matter the circumstance, if a set of guidelines is followed in a step-by-step process, putting all pieces together, whether in a theatre or emergency room, can convert a life-threatening situation into a lifesaving situation. A patient's smile when they hear the buzz of the fistula or watch the urine collect in the bag after a kidney transplant operation is an unforgettable feeling. I have been exposed to a wide variety of cadaveric/ live donor renal transplants and vascular access procedures. I hope to see myself pursue my career in transplant surgery, become a specialist renal transplant surgeon, and work in an institute that will provide a fillip to my professional career. I also wish to involve myself and get exposure to global surgical practices and help spread awareness about the benefits of organ transplantation and break all stereotypes revolving around it. I am sure that my surgical background, international clinical experience, strong communication skills, and passion for surgery will make me an ideal candidate for fellowship in renal transplant surgery at your esteemed institute. Brief role overview

1. Manchester Royal Infirmary (MRI) is a 750-bed teaching hospital situated next door to one of the country’s largest and most prestigious Universities with a vibrant medical school. 2. The transplant unit at Manchester Royal Infirmary (MRI) is the largest renal transplant programme in the UK, undertaking greater than 300 kidney transplants per annum, of which about 100 are living donor transplants. The unit is also the second largest unit in the UK undertaking solid pancreas transplants and has successfully undertaken islet transplantation since 2009.

3. The role of the post is to support the large clinical service in General Surgery and Transplantation. It is expected that the job provides good experience in all aspects of renal and pancreatic transplantation, renal-related surgery, and in multi-organ retrieval and General Surgery and endocrine surgery and become an integral part in the Manchester organ retrieval team.

4. The role is accountable to the Clinical Lead, and will also be expected to work closely with colleagues at all grades in nephrology.

Assist and perform complex vascular access surgeries like: AV Fistula, Graft fistulas, Brachio-basilic fistula, Radio-cephalic fistulas, transposition fistula, and Latest Vas-Q placement for dialysis and manage complications of the fistula like aneurysm, bleed, and stenosis with the help of Interventional radiologist team.
