Care Coordinator training began on 5/5/22; afterwards, immediately went into production. This job has many responsibilities. As a nurse triage I take calls from people with many different types of insurance or health plans. We obtain all relevant PHI, take description of why the pt/member needs to speak with the RN and submit the triage for call backs. Doctor location assistance for patients/members that are trying to find a new provider or change the one they are currently assigned. For facilities that have the option, we page out for many types of specialists for the patient/member to speak one on one w/their physician or NP. We assist with overflow or after hours calls for many health care plans or insurance companies. These calls can range in service needs of billing questions/payments to filling out communication forms for their Member Services Team to contact them, and assisting with appts when this service is available. In some cases we are contracted to provide further assistance with regards to health plans. All calls are of the utmost importance which require strict privacy with regards to HIPPA. Empathy as well as professionalism are extremely important.

2 monitors are used constantly and interchangeably. Zoom meeting held regularly with team supervisor and manager. Zoom is used for other types of meetings and training updates. Accessing translators is expected.

Windows applications used.

Must maintain quality high speed internet thru wired connections. No wireless or bluetooth accessories.

Once we have earned credentials (for which many facilities require), we must maintain those credentials and assist with calls for other care coordinators who are new and are working on obtaining those credentials. I choose 'blocks' or times I am available to pick up pkgs and delivery them responsibly using my vehicle. Upon arrival to the Hub, I scan and load all pkgs from my itinerary according to the number of stops.

We are to follow All customer directions using safety first. Requires ability to follow the route (itinerary), GPS, customer requests for locations of delivery (i.e. signature required?, lock box w/a

code?, contacting the customer via text or phone if the area seems dangerous or directions are unclear). Using the Flex app, a picture is normally required for the customer in order to show the exact location of pkg, proof of delivery and that any & all specifications were followed. REASON FOR LEAVING

Due to circumstances beyond my control with a family illness (covid related), I resigned.

I felt it was in the best interest of the company to allow for them to hire someone to complete the training. This was a great opportunity. I hope to continue to provide customer assistance, advocate for patients, assisting caregivers and medical personnel whether thru insurance, prescription services, or other avenues.
