Case Investigator CT2 (Contract Position)

Began training as a Contact Tracer on 5.11.20; went live as Contract Tracer on 5.14.20. On 5.25.20 original team disbursed to various HCPH sites, and I was joined by a team fresh out of training. Our Lead asked that the new team shadow me I was asked to train them. The following week I was promoted to CT2-Case Investigator- Special Investigations – completed the additional training, now my duties are:

• Processing Variant, Reinfection and Vaccine Breakthrough cases for congregate settings.

• Monitoring Data Tracking systems, to ensure all are processed in a timely manner.

• Preparing Documentation for CDC and DSHS.

• Obtaining Lab Specimens that meet DSHS guidelines.

• Initiate contact with positive cases, secure amicable relationship to conduct interview.

• Obtain history of possible place where Covid may have been present.

• Collect relevant contact information of Household and Social Contacts during infectious period; Inquire as to case needing assistance with essentials, if so pass to social working team.

• Offer access to Harris County assistance on ReadyHarris website, for retesting and relevant information.

• Explain how Isolation or Quarantine works.

• Provide contact information in case they would like to call back.

• Document interview on Database program and pass contacts on to Contract Tracers.

• Establish Contact with Directors at Facilities in order to assist management of the Covid-19 issues at the facility, requesting information and prep for assessment team.

• Updating health conditions of the Employees and Residents.

• Post case-filing, pre-prepare copy of the Plaintiff Fact Sheet (PFS) and Authorizations, send to client for completion. Follow up and calendar expected return of PFS and associated documents from client.

• Completed PFS’s over the phone, as necessary with clients who need help completing the documents. Finalized the PFS and Authorizations, sent typed version to client for approval or further updated changes. Bate stamped medical records with PFS and Authorization and submit Counsel in a timely manner.

• Responded to deficiencies from Defense and Local Counsel.

• Handled incoming client calls for information and assistance for specific dockets.

• Prepared reports, sorting data by status, injury or due dates also ran statistics on Various Cases.

• Sent out release letters, or status letters as required for various dockets, and notating relevant files in Database.

• Assisted in the Texas Bar Advertising application process, followed up with Advertising Team on Responses, assisted with the replies to State Bar Responses.
