EXPERIENCE IN MANAGEMENT OF HEAD TRAUMA:

This forms a major part of my practice given the huge number of accidents in my country. Operative mangement would consist of Craniotomy with evacuation of extradural, subdural, intracerebral hematomas, excision/elevation of depressed fractures. I have performed more than 2500 such surgeries for trauma since I have started private practice.

Non-operative treatment of head injuries would include ICP monitoring and neurointensive care.

EXPERIENCE IN MANAGEMENT OF SPINAL DISORDERS

1. I am well versed in treatment of all kinds of spinal injuries ranging from craniovertebral junction to the lumbosacral spine. I have operated on more than 1200 patients of spinal injuries independently. These include

Anterior approaches such as anterior cervical approach, transoral approach for odontoidectomy, transthoracic, transdiaphragmatic, retroperitoneal approaches for the thoracic and lumbar spine. Corpectomy with bone grafting, use of spacers and cages with anterior instrumentation are routinely being done by me.

Posterior approaches for decompression and stabilization of the spine would include Lateral mass fixation for CV junction problems, Magerl’s transarticular screw fixation with fusion for AAD (More than 30 cases), cervical lateral mass screw fixation, Mosmiami pedicle screw fixation for thoracic and lumbar fractures.

2. I am doing laminectomy with discectomy for lumbar degenerative diseases including both microdiscectomy and few cases of endoscopic discectomy.

3. Anterior cervical discectomy for disc prolapse and corpectomy for OPLL form a part of my surgical experience. I have recently done two cases of anterior oblique corpectomy for cervical compressive myelopathy.

4. PLIF, Mosmiami/Steffee pedicle screw with bony fusion techniques have been used by me in treating Spondylolysthesis and spondylolysis

5. I have a special interest in the managemet of Tethered cord syndrome and have considerable operative experience of 85 cases. These would include Diastematomyelia, tethered filum, and lipomeningocoele with and without neurodeficit.

6. Operating on spinal tumors (extradural, intradural, intramedullary) and spinal infections commonly tuberculosis is another area of interest for me.

7. Vertebroplasty for osteoporotic fractures and neural foraminal block for management of noncompressive lumbar radiculopathy is also part of my surgical armanterium.

EXPERIENCE IN NEURONCOLOGY:

Tumor surgery would include all kinds of intra and extraaxial tumors of the brain. Though majority are intraxial gliomas, I am adept at operating skull base tumors like meningiomas, posterior fossa tumors, CP angle tumors.

Microscope assisted transphenoidal excision of pituitary tumors are done independently by me and a few cases of endoscopic transphenoidal excision were performed with the help of my otothinolaryngologist colleagues.

EXPERIENCE IN PEDIATRIC NEUROSURGERY

As mentioned earlier a major part of my practice consists of management of pediatric spinal problems with an emphasis on the tethered cord syndrome.

A vast majority of emergency admissions consists of pediatric and adolescent head injuries due to falls and motor vehicle accidents and I am adept at managing the same

Pediatric brain tumors like posterior fossa tumors are a signifcant part of the oncology work I do in my practice

EXPERIENCE IN NEUROVASCULAR SURGERY:

I did successful clipping of anterior circulation aneurysm in a few patients in the past. Excision of vascular malformations done independently and 3 cases of successful carotid endarterectomy for carotid artery stenosis were done with the help of cardiovascular surgeons.

EXPERIENCE IN STEREOTAXY:

I have undergone training for the same during my residency and my stint in Hinduja hospital, Mumbai and am well versed with the technique. This also includes frameless stereotaxy (Neuronavigation)

EXPERIENCE IN GAMMAKNIFE SURGERY:

I have learnt Radiosurgery in the management of various neurosurgical disorders under the able guidance of Dr. B.K. Misra, P.D. Hinduja hospital, Mumbai.

EXPERIENCE IN NEUROENDOSCOPY:

I have individually performed 10 cases of third ventriculostomies for the management of hydrocephalus, 8 cases of endoscopic biopsies from intraventricular tumors and 18 cases of endoscopic evacuation of intaventricular hematomas.

EXPERIENCE IN NEUROINTENSIVE CARE:

Right from days of my neurosurgical training I have been involved in the intensive care management of my patients. I have been the incharge of the neurointensive care units wherever I have worked. This includes the nonoperative and postoperative management of critical patients. Fluid and electrolyte balance, control of infections, ICP monitoring are part of the management protocols. Emergency procedures like tracheostomy (open and percutaneous), intercostal drainage insertion, endotrachal intubation, central venous catheter insertions are done by me independently.

TEACHING EXPERIENCE:

I was actively involved in teaching the DNB neurosurgery residents in my department while I was in Medical Trust hospital, which includes taking clinics, conducting seminars and hands on training in the operation theatre.

The three years of Senior Resident post in J.J. hospital and 6 months of senior resident post in Nizam’s institute of medical sciences, Hyderabad are teaching posts.

The two years of Registrar post in general surgery is also considered as teaching experience.

MANAGEMENT AND ADMINISTRATIVE CAPABILITIES.

During my stint at City hospital, Kolhapur, Maharashtra, I was actively involved in the day to day administration of the hospital and coordinating the work of all hospital employees with the consultant doctors. I was involved in managing the finances of the hospital and developing interpersonal relationships with the press, police and public.

I have conducted multiple public awareness programmes like seminars for the general public and appearing on the local radio channel to answer live the questions put forth by the lay people regarding neurological disorders.

Having been brought up and practiced in different parts of India, I have developed linguistic skills, which include conversing, reading and writing in English, Hindi, Marathi, Malayalam, Tamil, Kannada and Telugu. The same has helped me immensely in understanding my patient’s problems and provide them a personal touch in healthcare and to build everlasting relationships with my patients and colleagues.

I have been called as an expert witness in many Indian courts to opine about accidents, their consequences and subsequent disabilities sustained by the patients.

I have been actively involved in guiding my students and juniors in their presentations at conferences and dissertations.

I am also involved in intense academic discussions with my colleagues, peers, and seniors via the electronic medium and make arrangements for the conduct of the same.

career goals and statement of purpose

I believe in the saying Change is constant and you are never too old to learn. I feel every day is part of my learning process. If given a chance I would like to develop my surgical skills further in the specialty of spine surgery and pediatric neurosurgery at a center abroad dealing exclusively with these subjects so that I can use the same for the better management of my patients.

I am a warm, friendly, easy going person, but focused on the task ahead. I know there is no substitute for hard work to achieve success but at the same time I realize that in pursuit of personal glory and academic excellence I should not forget the end point of this pursuit that is the patient care. I feel management of neurosurgical disorders requires in an equal measure the personal touch and psychological support for patients as that of surgical expertise.

OTHER INTERESTS:

I love good food and good music. My taste in music varies from Old Hindi songs, international pop and rock, to world music. I find it a great rejuvenator and have a calming influence on me. I also like to read a lot especially fiction. Autobiographical books of great leaders have molded my thinking and the way I view the world.

Similarly classic inspirational movies with a social message hold great interest for me.

I believe that I am here for a purpose and that I can make a difference to the world we are living in.
