Summary

Detailed-oriented Data Entry Intake with accuracy and proven ability to retrieve the process of gathering information to determine what services would be best to help my clients in their recovery. As required by our contract with the State of Texas, I collected information on symptoms, experiences, behaviors, substance abuse, past treatment

(what worked, what didn’t), general health, family history, strengths, and support system. Sensitive and personal information was also sought only to assure proper care and services. All of this information was important in determining my clients diagnosis and an appropriate plan of care. Additionally, other processes necessary to our system were performed as well, such as going over client rights and privacy practices and getting consents signed to send for records of other providers. I was also able to function independently, manage assigned tasks and always maintain accuracy and consistency.
